Elisa, 11/14/2005: Chromosome rearrangement involving 3, 7 and 4 with a duplication of 3q and deletion on 4q, classical autism, severe global developmental delay, seizure disorder, reflux, severe food aversion/100% g-tube fed, dysphagia, right facial palsy, mild hypothyroid, asthma, severe bilateral hearing loss, myopia, astigmatism, Chiari type I malformation, multiple minor dysmorphic features/duplication 3q syndrome, intermittent sensory ataxia, hypoglycemia with illness, ASD/VSD closed, cleft palate repaired, h/o aspiration as infant. Meds: Prilosec, Benadryl, Miralax, Albuterol, Flovent, Bactroban, Lacrilube ointment, Synthroid.
Eliana, 1/26/2009: Bilateral congenital cataracts removed, aphakic glaucoma, strabismus left eye, ptosis left eyelid, feeding problems/food aversions, soy enterocolitis. Reflux and constipation resolved. Multiple issues in infancy including central apnea, lethargy, vertical nystagmus, horizontal end gaze nystagmus, MSPI. Discharged from Early Intervention
Meds: Miralax, Cosopt, Xalatan.
