Caleb 7/14/05 - Former 35 weeker: GERD, FTT, Chronic Constipation, Chronic gut pain, developmental delays, Dysphagia & Sensory Processing Disorder. NG tube for 8 months; G-tube (Bard) since 7/11/07.
